   The recommended effective treatment window for 2025 has closed.

   Use this form to apply for a one-year extension if you are a Ward D resident and plan to treat your boulevard ash tree/s during the next treatment window: May–July 2026.

   By submitting this form, you agree to have the boulevard ash tree/s treated by a licensed arborist during the recommended timeframe. Starting in August 2026, the untreated boulevard tree/s are subject to removal.

   *Private ash trees (removal and treatments costs) and boulevard ash trees (treatment costs) are homeowner's responsibility. The City is responsible for the removal of boulevard trees.
